Athletic Training

Pre-participation forms required by The University of Sioux Falls:

Health History and Physical Examination (required by all NEW USF athletes)
Medical History Update (required by all returning USF athletes)
Medical Release Forms (required by all USF athletes)
Authorization for Release of Medical Information Form (required by all USF athletes)
A copy of current insurance card (FRONT and BACK)

All athletes must have all these forms turned in before being allowed to participate in any intercollegiate activities at the University of Sioux Falls.
